In accordance with Georgia Laws 1951, page 867, I am pleased to submit this report concerning the activities of the State Medical Education Board of Georgia for the period beginning July I, 1978, and ending June 30, 1980.
For your additional information, the report was prepared to include the action taken by the Board at its meeting held on July 15-16, 1980, and includes the granting of scholarships for 1980-81 academic year and the approval of the budget for the operation of the Board for the 1980-81 fiscal year.

President of the Medical Association of Georgia and became an ex
officio member of the State Medical Education Board. Dr. Milton
I. Johnson, Jr. , immediate past President of the Mediqal A ss ocia-
tion of Georgia, continued to serve on the Board for a second year.
Dr. H. Calvin Jackson, Jr., Manchester, served the second year of
his third four-year appointment. Dr. David A. Wells, Dalton,

P URPOSE
Th e State Medi ca l Educati o n Boa rd was es ta blished in 195 2 for a twofold purpose: (I) to provide a n a d equ a te suppl y of ph ys icians in the rural areas of Georgia; a nd (2) to provid e a program of a id to promi si ng medical student s w ho do not have th e fin a ncial mean s to attend a medical college.
THE CONSTITUTIONAL AMENDME T C R EATING THE STATE MEDI CAL E D UCAT IO BOA RD
The Board was created by an Amendment to the Co nstituti o n of Georgi a. The 1951 General As se mbl y of Georgia a d o pt ed a reso lution providing for the Constitutional Amendment which was ove rwhelmingly ratifi ed by th e elect ora te in th e Novem be r ge ne ra l election in 1952.
The purpose of the A mendm ent was to bring a bo ut a n ade4 ua te s upply of physici a ns in the more spa rse ly popul a ted a reas of th e State by increas ing the number o f m edica l stud e nt s from Geo rg ia in m edical sc hool , and by indu c in g a suffi c ie nt numbe r o f them to practice medi cine in rural Georgia.
Th e Boa rd is co mp osed of five m e mb e rs : T he Pres id en t of the Medical A ssociation of Georgi a, th e imm ediat e past pres id e nt of th e Medical Assoc iation of Georgia, ex offici o, a nd three me mbers appointed by the Governor. The three a pp o inti ve me mbe rs h o ld office for a term of four years.
Th e Secretary and Chief Exec uti ve Offi ce r o f the Board is th e same perso n who se rves as Exec uti ve Secretary of th e Boa rd of Regents of the University System of Georgia.
The Board is charged with rece iving a nd ac tin g up o n al l a ppli cations for lo a ns or sc hol a rs hip s made by stud e nt s w h o a re ci ti ze ns and residents of Georgia, who desire to beco me doctors of medicine, and who have bee n accepted for e nrollm e nt in accred it ed m edi ca l coll eges within th e United States. The purp ose of the sc ho la rs hip is to assist recipi e nt s in obtaining a medi ca l education to e na bl e them to become lice nsed , practicin g ph ys icia ns in co mmuniti es in Geo r-
gia having a population of 15,000 persons or less . It is the duty of the Board to make a careful investigation of the ability, character and qualifications of each applicant and to determine hi s or her fitness to receive a scholarship. The investigation of the applicant includes a re port on the ability of the applicant or hi s parents to pro vide financial aid for him or her to attend medical school.
Each scholarship or loan (as it beco mes if it is repaid in cash) was initially in the amount of $5,000 over a four year period, paya ble $1,250 per yea r. A Constitutional Amendment in 1968 increased the amount of each schola rs hip to $10,000 over four years and increased the population of a qualifying community in which a doctor could re pa y by practice to 10,000 persons.
In 1976, the Georgia Legislature adopted, and the peo ple a pproved , an additional Amendment to the Constitution providing for an inc rease in the amount of the sc holars hip from $10,000 to an a mount not to exceed $15,000 over four years and provided for a n increase in th e co mmunit y popula tion from I0,000 t o 15,000 in which a doctor may practice in repayment of the scholarship. The Amendment also increase d th e int e rest rate from 4% to 9% from the d ate of each payment to the applicant, compounded annually for those who repa y their loa n in cash and who do not practice in fulfillment of their commitment.

The Board has the a uthority to cancel the co ntract of any applicant at a ny time for ca use d ee med s uffi cie nt by th e Boa rd . In 1978, the Legislature a dopt ed an Amendment to the Constituti o n pr ov idin g for a revi s ion of Article X, which rela tes to retireme nt sys te m s and edu ca ti o na l sc hol a rs hip s. This a me ndm e nt did not pa ss in th e November, 1978, Sta tewid e Referendum . It would have provided for future Amendments t o th e Act to be a ppro ved b y the General Assembly rather than by Constituti onal Amendment. It a lso would have a uthori zed th e Boa rd to gra nt sc hol a rs hips to qualifi ed applicants e nroll ed in two-yea r as we ll as four- yea r medical sc hools.

The State Medical Education Board grants scho larships (o'those
applicants who establish a financial need and who express a strong
commitment to practice in a rura,l area in Georgia havin'g a popula-
tion of 15,000 perso ns or less in repayment of their scholarships.

The 1952 Constitutional Amendmeht creating the State Medi.cal Education Boa rd provlde{that;a redp ierit may repay his schol~r
sni p either in services 'or'iri'cas'h at four percent interest. Unfortu-
nately, the number 'of eligible recipient i electing to repay in cash
under the 1952 Amendinent increased quring 'the past fewyears to
more than fifty percent of the total number of scholarships granted.
The 1976 Amendment strength ens the primary purpose of the pro-
gram by conditioning the award of scholarships upon the financial

need of the applicant and his or her commitment to practice in a community ofl5,000 population or less or in a facility operated by or under the jurisdiction of the Department of Human Resources or the Department of Offender Rehabilitation. Recipients who fail to practice must immediately repay their loan in cash with interest at the rate of nine percent (9%) per annum from the date of each payment to the recipient.
ANNUAL RENEWAL
The Board has adopted stringent procedures to insure that a greater number of scholarship recipients will practice in approved areas ( 15,000 persons or less). Annual renewal of scholarship awards are made only to those students who demonstrate a strong intention of meeting their obligation to the State by practice. Each spring the members of the Board and its staff interview each scholarship recipient to secure a reaffirmation of intent to practice. The renewal of a scholarship for each succeeding year is also based on written statements of reaffirmation and a continuation of financial need.
APPLICATIONS RECEIVED AND SCHOLARSHIPS GRANTED - 1978-80
The Board received 83 applications for the 1978-79 academic year. At its meeting on July 18-19, 1978, it awarded 38 scholarships and renewed 93 scholarships previously granted. At a called meeting on September 6, 1978, 6 additional scholarships were awarded. Of the scholarships awarded for the 1978-79 academic year, 2 were relinquished . A total of 135 scholarships were awarded or renewed for the 1978-79 academic year.
The Board received 68 applications for the 1979-80 academic year. At its meeting on July 17-18, 1979, 31 scholarships were granted and 92 scholarships were renewed. At a called meeting on September 5, 1979, 6 additional scholarships were awarded . Of the scholarships awarded for the 1979-80 academic year, I was relinquished. A total of 128 students received scholarships for the 197980 year.
For the 1980-81 academic year, the Board received 52 applications. At its meeting on July 15-16, 1980, it awarded 31 scholarships and renewed 92. At a called meeting on August 28, 1980, 7 additional scholarships were awarded. Of the scholarships awarded for the 1980-81 academic year, 8 were relinquished. A total of 122 students received scholarships for the 1980-81 academic year. Table I shows the number of applications received and scholarships granted by the Board during 1978-80.

R ESULTS
On Jun e 30. 1980, the Sta te Medi ca l Educati o n Boa rd co mpleted it s twe nt y-seve nth yea r of o pera ti o n. Th e res ult s have been gra tifying. In thi s twe nt y-seve n yea r peri od. 983 sc ho la rships have bee n a wa rd ed to yo un g me n a nd wo me n need in g fin a ncia l ass istance to attend medical college. Of the 983 students receiving awards, 647 ha ve completed their medical education and become "eligible to practice." In accordance with the Constitutional Amendment creating the Board , 288 physicians chose to repay all or part of their scholarships by practicing either in a small community or in an institution operating under the jurisdiction of the State Department of Human Resources. Three hundred and fifty-nine doctors decided to repay their scholarships in cash; many of these entered further training; some established practice in communities with a population larger than 15,000; several began practice out of the State. There are 131 scholarships recipients still in medical school; others are engaged in further training in internships and residencies, still others are serving in the Armed Forces. Table III shows the present status of recipients of scholarships from the beginning of the program in 1953 through June 30, 1980.

SELECTING AND SERVING RURAL COMMUNITIES
A list of communities in critical need of physicians is compiled by the Board each year. A letter of inquiry is mailed to the mayors and hospital administrators of the rural communities throughout Georgia. Information is requested concerning the size of the community, the surrounding trade area, types of industries, number of churches and civic organizations, recreational facilities and schools. Information is also requested about hospital and drug store facilities , and whether the town will support a doctor.
If a community has appeared on the most recent town list and has not secured a doctor, a copy of the previous list is forwarded to a community official for verification and additional information. When completed , this list is mailed to every doctor who will be read y to begin practice upon completion of his training or military service during the current year.
A major service of the Board is the preparation (annually) of a list of physicians who will be "ready for practice" in July of each of the succeeding two years. Included on the list are doctors who will complete their military obligation during that time, those who will complete their internships and those who will complete their residencies. This list is available upon request to any community or physician. It is the only such list available in Georgia and provides a beginning point for rural communities seeking physicians.
Many of the doctors on this list are invited to visit interested communities with a view toward establishing a practice. Sometimes a doctor will practice in several communities before he finds a place where he wishes to settle permanently.
Table IV below shows the total number of doctors repaying in practice and cash, and the percentage of the total repaying by each method.

Of the 288 doctors from July I, 1953, through June 30, 1980, who have elected to repay all or a portion of their scholarships by practice, 156 have repaid their obligation in full by practice to the State as shown in Exhibit "G". Doctors who are currently practicing are listed with their place of practice in Exhibit "F''. Exhibits "H" and "I" list the doctors who have repaid or are repaying by practice and cash.

The "Country Doctor Program," as it is frequently called, has been favorably received in Georgia. Doctors practicing as a result of the program are scattered throughout the State. Many communities would be without medical services today except for this program. Dedicated young men and women of great promise, otherwise financially unable to obtain a medical education, have been able to do so as a result of these scholarships. Those who practice in communities in critical need of medical services have also added to the economic, political, social and civic welfare of our State. Those who repay their obligation in cash contribute to the well-being of the citizens of Georgia, through public service and practice. The funds received from scholarship recipients repaying in cash are paid into the State Treasury for reallocation by the General Assembly.
Although the State Medical Education Board program has been in existence for 27 years, it often takes a scholarship recipient 15 years or longer to rep~ obligation to the State in full from the date he first receives his scholarship. Table V shows the length of time that may elapse between the time an applicant receives his scholarship and the time he has completed his contract.

"We plan on making Commerce our home and are in the process of buying another house here this summer. I was board certified in Family Practice this year. I probably average about 35 patients a day doing a general practice which includes pediatrics, obstetrics and surgery. My wife and I still appreciate your help with this scholarship. Thank you."
"We have built a new office and home here in Warm Springs. Have seen 2200+ patients (30-40 per day) and have about 25 OB's to

be delivered (have delivered 25 already). I keep 3-7 patients in our local hospital and due to the "competition" I have stimulated other doctors in the area to accept medicaid patients. We find many patients coming to us who used to drive 35-40 miles before for their medical care. Your aid has been most appreciated."
"I am currently seeing an average of 75-100 patients per week in four of the clinics in our systems. I am working closely with a multidisciplinary team including social workers, psychologists (bachelor level) , a clinical psychologist, a pharmacist, and a psychiatric nurse. Two part-time psychiatrists work in two of our clinics. In all we have a current active caseload of approximately 1200 patients. To say the least , we all stay very busy. I would welcome another full-time psychiatrist on the staff. In addition, I am engaged in the private practice of psychiatry one day per week. I see about I0 patients per week, including out-patients and in-patient consultations in the community hospital. My experience leads me to conclude that the majority of the communities in rural southeast Georgia could use more psychiatric coverage; however, an association with a community mental health center would most likely be necessary rather than full-time private practice."
"I am fairly busy here in my first year of practice in General Surgery, doing approximately 6-8 cases per week. We have obtained a full time Anesthesiologist and desperately need a Urologist, Orthopedist, and a Pediatrician."
"I am very happy working with 2 other physicians. I see about 30 patients per day, including OB. My wife and I are active in church and community affairs (she is the Lamaze instructor) . Elberton is certainly a wonderful place to live and practice. We need a Boardcertified General Surgeon."
"I continue to practice general surgery in Statesboro along with my partner. Our practice continues to grow and probably next year we will add an additional man to our practice. I see anywhere from 20-30 patients per day now. I am very pleased with my practice here and plan to continue to stay here. If you hear of anyone in Ear, Nose and Throat, or Pediatrics then we could certainly use them here."

A CONTINUING NEED FOR RURAL PHYSICIANS
During the 1970's the Joint Board of Family Practice along with the Georgia Academy of Family Physicians and the Georgia Department of Human Resources projected a significant need for additional family physicians in the State through the year 2000. The latest study conducted by the Joint Board of Family Practice in March, 1980, entitled Primary Care Physician Manpower Report, State of Georgia, 1980-2000, reaffirms this need with conclusions summarized as follows: The average age of the 1,815 primary care physicians now practicing in Georgia is 50 and will increase to 70 by the year 2000. There is a great need for younger physicians to enter the primary care specialties. Approximately 301 primary care physicians will retire between 1980 and 1990. An additional 557 are projected to retire between 1990 and the year 2000. The result of this trend along with expected increases in population, clearly indicates the critical need for primary care physicians in Georgia through the year 2000.
COMMUNITIES WRITE US
As previously stated, the State Medical Education Board prepares a "Town List" each year which contains information and comments from officials of communities with populations of 15,000 or less throughout the State that are in need of physicians. The following comments were received from officials in various communities this year that reinforce the continuing need for family physicians:
"There is a shortage of physicians in the surrounding area, and our area is growing rapidly. Since becoming a part of the Metropolitan Atlanta area, we have begun to experience a great influx of growth from Atlanta proper. We would very much like to have a physician become a part of our community and grow with us. We need a doctor who can envision a big population in a few years. The Appalachian Trail starts eight miles north and goes up to Maine. This is a beautiful green area with natural surroundings of trees, clear water, water falls , and clean air that is unspoiled by enemies of nature."

"Additional doctors would be welcomed and supported. The hospital has enough beds to support several additional doctors and many of our citizens are going out of town for medical care."
"People of this county are in dire need of a good doctor and good medical care. Our town has one permanent doctor that works about a total of 30 hours a week and doesn't accept medicaid. He has all the business he wants and will retire in the up coming year or years."
"Our county is the 4th most rapidly growing county in Georgia especially with the new Federal4-lane (1-575). Our area is in critical need of specialists. We truly have an excellent base for primary care doctors and we need specialists badly. Any specialist coming to this town will be on the ground floor of building something really exciting and growing. General Practitioners and Surgeons are the only medical area covered completely."
"We have need for any specialist. Currently one of our doctors is an internist while the others are general practitioners. The administrator of the hospital will be glad to host a prospective medical practitioner desiring personal observation of community and facility."
"We feel that our county has need of a young doctor to replace the services being rendered by the doctors in the oldest age group who are not able to devote as much time to medical practice as formerly."
"Our community needs at least one if not two doctors very badly and our local doctors would welcome additional help with their patient load."
"Please come look our city over, we offer an excellent place for you to help sick people, rear your family and become wealthy."
"Our town is an exceedlingly fast growing community and the community's addition of physicians is in no way keeping up with the pace of the city's population growth. Any young medical student graduating from medical college will find us an exceptional community in which to establish his or her practice and will be richly rewarded for their decision.
"New office space available with financial assistance for relocating and setting up new practice."

TRAINING PHYSICIANS FOR FAMILY PRACTICE
The Board of Regents recognized the diminishing number of physicians in primary medical care over the last three decades , and at the request and urging of the State Medical Education Board, a Department of Family Practice was established at the Medical College of Georgia in1972. Dr. Jon C. Calvert, Chairman of the Department of Family Practice, cites the following information concerning the development of the Family Practice Preceptorship and Family Practice Residency Training Programs. All scholarship recipients are required to participate in the Preceptorship Program and strongly encouraged to select a Family Practice Residency.
"There are presently five Family Practice residency training programs in the State of Georgia: Augusta, Columbus, Macon, Rome, Savannah. Under the direction of Mr. David Satcher a sixth program will begin at Morehouse School of Medicine in Atlanta beginning July, 1981. In addition , Northside Hospital in Atlanta is actively considering establishing a Family Practice residency training program.
It is equally important to provide medical students an opportunity to be exposed to positive experiences in Family Practice while in medical school. The Department of Family Practice at the Medical College of Georgia is responsible for teaching medical students in four required courses: Introduction to Family Medicine, Physical Diagnosis, Community Medicine, and Clinical Medicine. The Department of Community Medi-

cine in Family Practice at Morehouse School of Medicine is responsible for teaching three required courses to its medical students : Human Values in Medicine, Community Medicine and Family Health, and a required preceptorship of one-half day a month for each student.
Through support of the Georgia Academy of Family Physicians and funding from the Joint Board of Family Practice, preceptorship opportunities (students spend one month with family physicians in private practice) are offered for medical students from Emory University School of Medicine, Morehouse School of Mec;iicine, and the Medical College of Georgia. Currently there are 93 family physicians in private practice throughout the State of Georgia who serve as preceptors. During the past year 126 students from the Medical College of Georgia participated in these preceptorships mcst of them between their freshman and sophomore year. In addition four students from Morehouse participated in these preceptorships, one from Emory and two from medical schools outside of Georgia.
The Georgia Academy of Family Physicians has been instrumental in developing Family Practice forums at Emory Uni versity School of Medicine, Morehouse School of Medicine and the Medical College of Georgia. Through these efforts an increasing number of students have expressed interest in Family Practice. Last year 15 per cent of the graduating class from the Medical College of Georgia entered the speciality of Family Practice which was above the national average. One student from Emory entered Family Practice. Morehouse School of Medicine had not yet graduated its first class.
Through July of 1980 there have been 80 graduates from the Family Practice Residency training programs in Georgia. Of these graduates 45 have established practice in the State of Georgia or a retention rate of approximately 56%. The national average is between 55 % and 65%. In Georgia, 27 different communities have been selected by these 45 residents as their practice sites. There are currently 94 residents in training in the five residency programs in the state.
The State Medical Fair which was held for the second year in Augusta was well attended by Family Practice residents and residents from other specialities. This provided an excellent opportunity for residents and medical students to meet and talk with representatives from rural communities throughout the state who are interested in attracting physicians.

Aproximately 73 rec1p1 ents of the State Medical Education Board's Scholarship Program who attended the Medical College of Georgia from 197 8 through 1980 took part in either the Family Practice residen cy program, the preceptorship program or took an elective course of st udy offered by the Department of Family Practice. As indicated above there is in creased interest in activ ity at Emory University School of Medicine, Morehouse School of Medicine, and the Medical College of Georgia concern ing the speciality of Family Practice. The number of medical students interested in Family Practice continues to steadily increase and will in the years to come serve as a valuable resource of family phys icians for communities throughout the State of Georgia."

In 1979 the State Medical Education Board, along with the Medical College of Georgia and the University of Georgia Cooperative Extension Service, sponsored the first annual Medical Fair. The purpose of this Fair was to bring together resident physicians, medical students, dental students and their spouses with representatives from communities in Georgia of 15,000 population or less , having an approved hospital, to discuss practice opportunities around the state. In 1980 the Joint Board of Family Practice and the Medical Association of Georgia also served as sponsoring organizations.
During the fall of 1979, and again in 1980, the. Fair was held in Augusta and has grown in size and interest, attracting more than two hundred seventy-four (274) representatives from fifty-four (54) individual Georgia communities and more than three hundred twenty-six (326) resident physicians, medical students and dental students. Placements resulting from contacts made at the 1980 Fair by thirty eligible resident physicians will not be known until their medical training is completed in July, 1981. The 1979 Fair resulted in the placement of four resident physicians who completed their training in July, 1980. In addition, two dental students located in Montezuma and Ashburn as a direct result of contacts made at the 1979 Fair.
In preparation for this event each year, an orientation session is held in the spring with community representatives to discuss preparations for meeting resident physicians at the upcoming Fair. Special meetings are also held with resident physicians prior to the Fair to discuss and share information on how to make successful contacts and select a community in which to practice.
The contact and placement possibilities offered by the Fair are exciting. Certainly, it has provided for physicians and community representatives alike an opportunity for a significant and meaningful interaction with one another.
With the assistance and continuing encouragement of these outstanding individuals, communities, and groups, including the Medical College of Georgia Foundation and the Georgia Municipal Association, we believe that this annual event will have an increasingly strong and positive influence on the location of physicians in rural communities.

THE COST OF THE PROGRAM
During the past two years, the amount of money received from scholars hip recipients who repaid their scholarships in cash was $443,433.96. Many of these scholarship recipients who have repaid in cash remain in Georgia, although they may not practice in a qualifying community. The amount of scholarship funds paid to recipients in medical school during the past two years has been $792,000.00. We submit that the value of the program far exceeds its cost in dollars. Indeed , it is an investment in the future of Georgia's rural areas. An investment which , in our opinion provides the greatest dollar return for Georgia taxpayers of all fund s appropri ated by the Ge nera l Assembly.

As stated elsewhere in this report , the Consitutional Amendment which was adopted on November 2, 1976, authorized an increase in the amount of each scholarship from $10,000 to $15,000 payable over a four-year period. However, legislative funds were not immediately available for the full increase to $15,000 for all scholarship recipients. Hence, the amount of each scholarship for first-year recipients was increased gradually one year at a time beginning in 1977. Under this plan, the 1979 scholarship recipients were the first ones eligible to receive the full $15 ,000 scholarship, payable at the rate of $3,750 annually if approved by the Board.
CONCLUSION
The members of the State Medical Education Board and its staff wish to express their appreciation to the Governor and members of the General Assembly for making this scholarship program available to hundreds of outstanding young men and women. We are confident that in the years to come the taxpayers of Georgia will reap an outstanding harvest of better rural health care for the underserved medical areas of our state.
