The Georgia State Department of Education has made available the
resources of its AudioVisual Service to the public libraries of the state
that can qualify for this service A special group of films for public li
brary use has been added to the collection of films for schools Public
Drariea may borrow from both collections
It is the objective of the public library service to lend films
to county or regional libraries for use in their own library programs and
for loan to responsible organized groups who are following a serious ed
ucational program and turn to the library for program materials Local
county or regional libraries giving film service to organizations should
work out agreements with those organizationso Such agreements should
establish responsibility for use and care of films
In addition to this catalog libraries giving film service to
their communities will need to have available certain basic film lists and
film rental library catalogs The following which may be purchased with
state aid funds have proved to be helpfuls

INSTRUCTIONS FOR REQUESTING AND USING FILMS
1 Fill in application form for use of these films LAVl and mail with 1000
annual fee to
State Department of Education
AudioVisual Service

2 Make requests only on the official blanks AV2
3 Requests may be made in one of two ways
a Make requests for a full calendar ronth all at one time and be sure that
they are mailed not later than the 20th of the month preceding
b Make requests twice a month in the following manners
Requests from the 1st through the lth of the month must be mailed no
later than the 23rd of the preceding month Requests from the 16th of
the month through the 31st of the month must be mailed no later than
the 7th of that same month
U Start with the first date that you wish to use films and follow a progressive
date sequence to the end of the period for which you are making your requests
Do not mix the months or the dates
5 Up to five films can be books for a period of from one to thirty days with no
more than five to be held at one time Always specify the length of time wanted
6 Librarians or coordinators must sign each request with complete street ad
dress and town Requests cannot be accepted from anyone else
7 If damage occurs return the film to the above address at once for repairs
8 DO NOT REWIND THE MATERIAL
9 Always return the films as booked and in the same cases in which they are
received
10 Leave the shipping tickets in the cases
11 When it is necessary for films to be cancelled always return the confirmation
and check the space at the bottom of the blank indicating cancellation
12 Always return films promptly so as to reach us by the due date and never hold
over for any reason unless permission is granted by the AudioVisual Service
13 All films must be returned on serviceable reels of the same size as received
on These must he straightsided reels

Film service to public libraries in Georgia has increased steadily
This makes it even more important to have some measurement of both its extent
and its effectiveness The keeping and reporting of statistics will provide
some measure of the extent and effectiveness of this phase of the library
program
On the reverse of this sheet is the first page from the PUBLIC
LIBRARY FILM STATISTICS collected and distributed by the American Library
Association The items which it calls for recur constantly in questionnaires
to the state office Our program cannot be evaluated properly on either the
local or national level unless this information is available
Georgia Libraries are urged to gaher this information as films
are used This will expedite the periodical reports to the agencies which
gather statistics
