State of Georgia Refugee Health Program
The purpose of the State of Georgia Refugee Health Program (SRHP) is to promote the physical, mental, and social well being of all newly arrived refugees in the state of Georgia. Refugees, Asylees, Parolees, Cuban/Haitian entrants and Victims of Human Trafficking are eligible for the program. Local county health departments provide the refugees with domestic health screenings. The program ensures that refugees receive adequate healthcare. The program is funded through Refugee Medical Assistance (RMA) and covers the refugee's health care needs for up to 8 months from their date of arrival in the United States (U.S).
The Georgia Refugee Program requires that Refugee health screenings begin within 30 days from their date of arrival. RMA will not cover screening costs that begin after 90 days. This aids in the early detection of contagious diseases as well as reducing the spread of disease. In addition, outreach and educational services to the refugee communities, further ensures disease prevention.

Number of Arrivals 3,378 Refugee New Arrivals entered Georgia in Fiscal Year (FY) 2010. Refugees arrived from 60 countries. The largest numbers arrived from Burma, Bhutan, Iraq, Ethiopia and Somalia. See Chart 1

County of Residence in Georgia The largest resettlement of Refugees was in DeKalb County. (2997) The next largest resettlements were in Fulton (100), Gwinnet (81), Madison (49), Cobb (31), Colquitt and Cherokee, each with (19).

Timeframes for the Refugee Screening Process
Of the 3,167 new arrivals screened in FY 2010, 647 were screened in 30 days, 1,248 screened in 60 days, 1,141 screened in 90 days and 131 were screened after 90 days. Only 20.4% of those screened were screened within 30 days. Exceeding 30 days for the initiation of screening endangers the health of the refugees who may have unknown health problems. Refugees only have 8 months of Refugee Medical Assistance from their date of arrival. Should an illness be found, screenings exceeding 60 and 90 days could mean that Refugees may not have sufficient time for follow-ups or medical treatments before their medical assistance runs out. If the Refugee has a previously unknown contagious disease, the longer it takes to screen the Refugees the more chance there is of the contagion spreading to the general public.
Health screening results indicate that 351 Refugees had abnormal test results for TB skin test, 84 tested positive for Hepatitis B Surface Antigen, 112 had abnormal test results for intestinal parasites, 1 tested positive for HIV and 561 had abnormal test results for non-communicable diseases such as hypertension, diabetes, malnutrition, anemia, visual and dental problems. All were referred to respective physicians and clinics for treatment and follow-up. All Positive TB and HIV findings were reported to state TB and HIV programs. 211 refugees were not screened because they either refused to be screened or due to migration to other states. Cost Analysis Refugee Cash and Medical Assistance (RCMA) funds this service through the Georgia Department of Human Services (DHS), Refugee Programs Unit. The Unit developed a Memorandum of Agreement (MOA) with the Department of Community Health and Division of Public Health, for direct service provision.
The average cost per client screened was $100.41. The following is a breakdown of expenses: $212,415 Administrative, $4,789 Travel, $5,000 Regular Operating Expenses and $318,013 Direct Services to clients. (Screening and Referrals) See Chart 4

Issues and Recommendations Exceeding the state's Refugee Program 30 day standard for the initiation of Health Screenings may potentially endanger the Refugee's health. Community members may also be exposed to an unknown contagious disease for a longer period of time than necessary. For FY 2011, the state Refugee Program requested a 25% reduction of new arrivals in part, due to the over saturation of DeKalb County and the inability of the DeKalb Health Department to screen Refugees within the first 30 days from their date of arrival. Table 4 illustrates that the problem with screening clients in their first 30 days is not just a problem of overcrowding. The State Refugee Program is requiring that health screenings be initiated in the first 30 days after the Refugee's arrival date or the Department of Community Health (DCH) will not be able to charge the service to Refugee Medical Assistance (RMA). Also, Voluntary Agencies are required to schedule appointment for newly arrived Refugees shortly after their arrival, so screenings can be done in 30 days. Even with the action the state Refugee Program put in place to insure that screenings take place within 30 days, the state can not control the fact Asylees, Parolees and Victims of Human Trafficking schedule their own screenings and account for 30% of those screened after 30 days. Refugee Screening invoices had a delayed of 4 or 5 months before they were submitted for payment. With the Memorandum of Agreement (MOA) for Fiscal Year 2011, the State Refugee Program is requiring that DCH change their billing process to require that services provided in one month are reported and submitted for payment by the following month. Due to a major change in policy by the Office of Refugee Resettlement (ORR), requiring that RMA only cover expenses not covered by Medicaid, the MOA has been put on hold until ORR approves Georgia's new health screening cost rates.
