Georgia HIV/AIDS Take Action, Keep Educated
(TAKE) Project
Overview The Take Action, Keep Educated (TAKE) Project, in the Georgia Department of Community Health's (DCH) Office of Health Improvement (OHI), focuses on raising HIV/AIDS awareness and improving the continuum of HIV/AIDS treatment, care and prevention services for minority populations in Georgia.
Mission of the Project To reduce HIV/AIDS disparities in prevention and treatment among Georgia's minority populations.
Project Goal To create a more unified approach to addressing minority HIV/AIDS care and prevention issues at the state, county and local levels.
Objectives of the TAKE Project To increase the awareness of HIV/AIDS disparities in Georgia's minority communities

Georgia ranks sixth in the nation in AIDS cases
Minorities are more adversely affected by the disease
Education and preventive actions are vital to stopping the spread of HIV/AIDS
Quality care and treatment services can sustain a healthier life for people living with HIV/AIDS
To build collaborations to address HIV/AIDS disparities in treatment, care and prevention services
To increase, develop and improve the care service delivery capacity among minority and minority serving community-based organizations
To inform Georgia's population that education and preventive actions are key to stopping the spread of HIV/AIDS
TAKE Programs
Communities of Practice (CoP)
The CoP is comprised of public agencies, Hispanic/Latino and community and faith-based organizations. The goal is to strengthen, formalize and foster collaboration through shared knowledge and communication that has an impact upon the HIV/AIDS continuum of treatment, care and prevention services in Georgia.

(TAKE) Project
Project Stronger Together (PST) PST is a collaboration between the OHI HIV/AIDS TAKE Project and three community partners: AID Atlanta, Mary Hall Freedom House and Wholistic Stress Control Institute. PST provides technical assistance to increase, develop and improve the continuum of HIV/AIDS prevention, care and treatment service delivery capacity of minority community-based, minority-serving organizations in metropolitan Atlanta and surrounding areas.
