How can I enroll as a provider in the VFC Program? Enrolling in the VFC Program is easy! Call Vaccines for Children/Georgia Immunization Office at (404) 657-5013 or (800) 848-3868 (outside metro Atlanta).
You should: Request a Provider Enrollment Package,
which lists Georgia's requirements for participation
Complete and return the Enrollment Form
Return the Provider Profile Form to ensure you receive the amount of vaccine needed for eligible patients

What is the Vaccines for Children (VFC) Program? The Vaccines for Children (VFC) Program is a federal program that provides eligible children routinely recommended vaccines at no cost. The federal government and the State of Georgia pay for the vaccines, and doctors and clinics agree to give the vaccines to children who qualify. You may, however, be charged a small fee for administration of the vaccine.
Who is eligible? In Georgia, children from birth through 18 years of age can receive vaccines through the VFC Program if they meet at least one of the following requirements: Eligible for Medicaid No health insurance American Indian or Alaska Native Under-insured (have health insurance, but
vaccines are not a covered benefit) Eligible for PeachCare for Kids

What are the benefits of the VFC Program?
The VFC Program
Provides no-cost vaccines to eligible children, saving parents and providers out-of-pocket expenses for vaccine purchases
Enables infants and children to receive vaccines from their primary care provider
Reduces the risk of serious illness or even death from vaccine-preventable diseases
Enables vaccine purchase at lower pricing through use of federal vaccine contracts

FOR PARENTS
How much will I have to pay?
You won't pay anything for the vaccine itself, but you may be asked to pay a small fee for administration of the vaccine. You can still get the vaccine, even if you are unable to pay this fee at the time of your visit.
Where can I get VFC vaccines for my child?
If your doctor is a VFC-participating provider, you can get your child's shots in the doctor's office. However, if your doctor is not a VFCparticipating provider, you can take your child to your local public health department for vaccines.
Most pediatricians and family physicians participate in the VFC Program. You can ask your doctor or call the Georgia VFC Program to locate VFC-enrolled providers in your area.
