The following criteria will be utilized to determine clinical appropriateness for admission: There is the ability to improve health status The medical complications/co-morbidities involved
have caused a significant decline in physical functioning The patient requires a minimum of 8- 12 hours of nursing care per day The patient could benefit from the involvement of a comprehensive interdisciplinary team There is an anticipated discharge disposition The expected length of stay is greater than 14 days The patient is 18 years of age or older The patient has been screened and approved for admission via the pre-admission screening assessment process (provided at no cost) The patient's disability is of recent onset or recent progression The patient is medically stable for transfer
Each patient referred to Roosevelt Warm Springs Long Term Acute Care Hospital is carefully evaluated for admission to assure they meet hospital acute care admission criteria, and to make sure Roosevelt Warm Springs provides appropriate care for their particular condition. Anyone may make a referral; however, a physician order is required for admission. Call 706-655-5254.
Inpatient Rehabilitation
Physical Medicine and Rehabilitation speciali t (physicians) direct the Roosevelt Warm Springs Rehabilitation Hospital. Historically, "The Roosevelt Way," a comprehensive approach to inpatient rehabilitation, has demonstrated significantly higher functional independence gains and di charge goals than existing national averages.

Admissions
The inpatient admission process begins with a referral to either our Inpatient Rehabilitation Ho pital or to our Long Term Acute Care Hospital. Anyone can inquire about making a referral; however, a physician order is necessary for admission. Roosevelt Warm Springs admits and treats patients who are at least 16 year old, without regard to race, color, national origin, religious creed, sex or sexual orientation. The same requirements for admission are applied to all patients.
If the referral is received from an acute care hospital, a clinical liaison will visit the patient and family to determine each patient' pecific medical and rehabilitative needs to assure the patient receives the most appropriate care for their particular medical/rehabilitation condition. During this evaluation the patient and their families are informed about the services available at Roosevelt Warm Springs and an explanation of the admission process for both hospitals (Inpatient Rehabilitation and Long Term Acute Care). Patients referred from facilitie other than acute care hospital or from home are first scheduled to be seen by our Outpatient Physician Services.
Upon admission to Roosevelt Warm Spring, ho pital patients need to have their insurance card and a family member available to assist with the admission process.

Conveniently represented by therapy clinic throughout West Central Georgia, Outpatient Services is a major component of the comprehensive rehab offered by Roosevelt Warm Springs.
Outpatient Therapy Clinics Along with Warm Springs, laGrange, ewnan and Griffin are home to Roosevelt Warm Springs Outpatient Therapy Clinics, with each facility offering a broad spectrum of treatment programs that provide individualized therapy for both adult and pediatric patients. Physical therapy is available at all locations with occupational and speech therapy also available in Warm Springs and laGrange.
The therapists in each clinic work clo ely with referring physicians to develop individualized programs designed to assist each patient in achieving maximum functional gains in a minimum number of visits. Education is emphasized to help patients understand the importance of ongoing rehabilitation.

Acupuncture Under the leader hip of Dr. Duan Huang, M.D., PhD and a board certified acupuncturist, Roosevelt Warm Springs offers the alternative medicine acupuncture for pain management and a variety of other diagnose .
Aquatic Program The Roosevelt Warm Springs Aquatic Program has two options ideal for individuals with chronic pain, spacitisity, arthritis or decrea ed range of motion.
Aquatic Therapy - This service requires a physician's prescription and is a medically based treatment utilizing the "Warm Springs Water." It is conducted by a licensed physical or occupational therapist.
Aquatic Exercise Classes- These classes are focu ed on general exercises. They are designed so individuals can use the "Warm Springs Water" in a group setting with an activity therapist.
Diabetic Foot Center The Roosevelt Warm Springs Diabetic Foot Center provides outpatient podiatric services with special emphasis on the prevention, education and treatment of diabetes and related foot problems. A staff podiatrist and team of specialists provide foot screenings and treatment plans for every patient. Services include: diabetes education, footwear, wound care, skin and nail care, total contact casting, surgical services and the treatment of charcot fractures. DFC services are also available upon request at the RWS Newnan Outpatient Therapy Clinic.
Outpatient Physician Services Roosevelt Warm Springs Outpatient Physician Services is a consulting service, accepting referrals from other physicians. Staff physicians also provide follow-up care for patients who have been discharged from inpatient settings.
Post-Polio Services Roosevelt Warm Springs is recognized as a specialty center by the International Polio Network and offers comprehensive post-polio services. Services can include evaluation, treatment, muscle testing, functional evaluation, therapy, orthotics and education about managing health problems related to the effects of polio.
Rehabilitation Technology The Roosevelt Warm Springs Rehabilitation Technology Department offers custom seating and wheeled mobility products. It provides durable medical equipment that includes bath aids, ambulation devices, custom crutches and assistive technology. Custom orthotics and prosthetics are also available.
Seating & Wheeled Mobility Roosevelt Warm Springs Seating & Wheeled Mobility is a service designed to meet specialized needs of individuals who require wheelchairs as their primary means of mobility. A seating team evaluates clients, utilizing the latest technology. Services include wheelchair selection and fitting, custom seating and alternative control interfaces for power chairs.
Wound Care With is own certified wound, ostomy, countinence nurse, Roosevelt Warm Springs offers specialized care for diagnoses involving these health issues.
Financial Options
Roosevelt Warm Springs will accept payment by way of cash, check or major credit cards. Our Billing/Collection Office is open 8 a.m. until 5 p.m., Monday through Friday.
Upon receiving inpatient or outpatient care, our Admissions staff will ask you several questions concerning insurance coverage and payment choices. It is very important that we have accurate information. If any subsequent changes occur that will affect your coverage, our Billing/Collection Office should be notified as soon as possible. On all outpatient services, you will be expected to pay your portion of the bill at the time services are received.
Commercial Insurance Roosevelt Warm Springs accepts commercial insurance assignment and will file a signed claims with your insurance carrier(s). Your insurance coverage will be verified and, for inpatient care, it must provide a minimum of 75 percent of your anticipated services. This filing is done as a courtesy to you and does not constitute agreement by RWS to accept your insurance as full payment. Once your insurance has paid its portion, applied the amount to your deductible or denied all or part of your bill due to policy limitations, a statement will be sent indicating the balance you are responsible to pay. Payment for all services is expected within 30 days from the first statement date unless other arrangements are made. ot adhering may re ult in the requirement that outstanding balances be paid before any further services can be rendered. Should the combined payments from patient and insurance result in a credit balance, the account balance will be refunded in an appropriate way.
Private Pay Outpatients Payment is expected at the time services are received. If any balance remains, a statement will be generated indicating the amount you are responsible to pay. Payment is expected within 30 days of the first statement date unless other arrangements are made for an extended payment plan. Not adhering to RWS financial policies may result in the requirement that all outstanding balances be paid before further services are received.
Medicare Patients Roosevelt Warm Springs accepts a~signment on all covered Medicare charges. Upon payment of the allowable charges
by the Medicare intermediary and carrier, Medicare patients wiU be billed for the remaining balances and noncovered charges. Insurance will be rued with the Medicare patient's supplemental carrier (if any); however, any unpaid balances are expected to be paid by the patient within 30 days of the first statement date unless arrangements are made for extended payment.
Medicaid Patients Roosevelt Warm Springs Medicaid patients must show proof of current Georgia Medicaid eligibility (Medicaid Swipe Card). Medicaid patients will not receive monthly statements except for services which are non-covered and/or for required co-payment by Medicaid.
Worker's Compensation Validated Worker's Compensation services at Roosevelt Warm Springs are billed either to the employer or the employer's Worker's Compensation carrier, depending on company policy. In the absence of validation by the employer of a work-related injury, the patient will be held responsible for payment of services rendered. Should a validated Worker's Compensation service subsequently be denied by the employer or the Worker's Compensation carrier, such charges will be the fmancial responsibility of the patient.
Indigent Care Policy Roosevelt Warm Springs provides inpatient services free of charge to qualified patients who have met the RWS criteria for indigency. RWS does not provide outpatient services free of charge for ancillary services, durable medical equipment, orthotics or prosthetics. The Roosevelt Warm Springs Indigent Care Fund provides programs for qualified patients who have met the RWS criteria for indigency and can receive inpatient and outpatient services free of charge.
For Roosevelt Warm Springs inpatient care, a legal Georgia resident may apply for free services by requesting and completing an Application for Uncompensated Services. Those persons desiring to make application for indigent care should contact the RWS Admissions Office. AU persons approved for uncompensated services who have formerly applied for Georgia Medicaid before they are admitted must apply for admission. Those persons whose Medicaid is ultimately denied will receive free care as an indigent under this policy.

Long Term Acute Care
The Roosevelt Warm Springs Long Term Acute Care Ho pital is under the direction of Internal Medicine specialists. This hospital specializes in the treatment of medically complex patients who are suffering from a cata trophic or chronic illness. Due to the complex nature of each patient's condition, the majority of patients will require an extended stay in our acute care environment, an average of 25 days or more.
Our focus i to return patients with multi-system medical problems to their highest functioning level. This is achieved through our interdisciplinary team approach, which measures progress by daily monitoring and planning, and focuses on outcomes. Our interdisciplinary team includes Internal Medicine physicians, nurses, technicians, case managers, therapists (physical, occupational, recreational, speech, respiratory) pharmacists, psychologists and ancillary service technician such as x-ray and lab attendants. Patient goals are a large part of every individualized treatment plan and our team works as a unit to achieve as many of their goals as possible in the least amount of time. The families of our patients actively participate in the team concept and remain at the center of the restorative process. Patient and family education is ongoing throughout the stay.
The goal of Roosevelt Warm Springs Long Term Acute Care Hospital is to maximize the recovery potential of every patient we serve. This is accomplished through services for wound care management of skin disorders, management of infectious processes, neurological issues, respiratory care and complex orthopedic issues. Our patients do require an active primary diagnosis and two active secondary conditions.
