Purpose of Study
The Centers for Disease Control and Prevention (CDC) and the Advisory Committee on Immunization Practices (ACIP) recommend that adolescents routinely receive Tdap, MCV4, and the 3 dose HPV series at 11 to 12 years of age1. Beginning in the 2014-2015 school year, Tdap and MCV4 were added to the list of required vaccines for Georgia students entering seventh grade who were born on or after January 1, 2002. This report summarizes the annual assessment of immunization coverage for Tdap and MCV4 as well as one and three dose coverage of HPV among seventh grade students in Georgia's 18 Public Health Districts. In addition to determining adolescent coverage rates for vaccines required for school entry, the GA Immunization Program is interested in determining whether providers are making strong, clear HPV recommendations to parents of adolescents when teens are seen in clinics for their seventh grade Tdap and MCV4 vaccinations.

Methods
Study Design The annual Georgia Adolescent Immunization Study (GAIS) employs a cross-sectional research design to ascertain the vaccine coverage rate for 7th grade children in the State of Georgia. Immunization history data for cohorts of children in the 7th grade from 18 Health Districts were analyzed to calculate these rates. Identifying information and immunization history data were obtained from school 3231 files and the Georgia Registry of Immunization Transactions and Services (GRITS). Immunization rates for Tdap, Polio, MMR, MCV4 (meningococcal), Hepatitis B, Varicella and HPV vaccines were calculated.
Data collection for the 2016 GAIS study began January 15, 2016 and lasted for three months. A two-stage cluster-sampling scheme was developed to randomly select the study sample. Stage one of the cluster sampling scheme involved randomly selecting up to 30 middle schools from each of the 18 Health Districts. The number of schools sampled from each district was determined using the following factors:
A) target population size per district B) estimated percent response rate C) estimate design effect D) confidence coefficient E) confidence interval width If a District had fewer than 30 schools, then all schools in that District were sampled.
Stage two of the sampling scheme involved randomly selecting an established number of students from each school. This number varied by District depending on the number of schools sampled and population. Basic coverage rates were then calculated for each of the specific vaccines of interest.
Target and Sample Populations The target population of the 2016 GAIS included all 7th grade children in the State of Georgia during the 2015-2016 school year. A sample of 6,191 seventh grade students, attending both private and public Georgia schools, were selected for the study. The sample design allowed for independent estimates to be calculated for each of the 18 Health Districts in the state.
Data Collection An electronic web-based data collection system named "GAIS" was created to systematically collect the required information for each child. The GAIS data collection system contains three distinct sections for completion: Child, Notes and VX List (Immunization History).
Data collection was carried out primarily by each Health District's Immunization Program Consultant (IPC). IPCs in each Health District participated in training at the start of the data collection period. A Training Manual was also provided and made available on the GAIS log-in screen.
Data Collection Protocol Step #1: Obtain student sample Before the data collection process began, IPCs were assigned a maximum of 30 schools in their respective Districts and assigned a required number of students per school to be sampled. Once at a school, the IPC obtained an alphabetized roster of all current 7th graders attending that school. Using the total number of seventh graders, a random number generator was applied to randomly select the 13 students chosen as the 2016 GAIS sample.

Step #2: Collect immunization forms at public and private schools. Once the IPC selected the school sample, each specific child's 3231 form was located to obtain their immunization history. A comparison of the GRITS record and the 3231 form was conducted for each child to make sure all vaccines received matched between sources. If any dates were missing from either source, steps were taken to update the 3231 form and/or the GRITS record to match. If a 3231 was found to be expired, a new one was printed out from GRITS for school record.
Step #3: Input children to data collection website A record was then created by the IPC for each child in the GAIS website. Information collected included first and last name, date of birth, county, school, exemption and waiver status and lab confirmation of immunity. A blank vaccine list was also available in the GAIS record for the IPC to either: 1) add each of the vaccine dates of interest in the table; or, 2) perform a direct upload from GRITS into the GAIS record.
Step #4: Data returned to State Epidemiology office Using the GAIS system, data collectors completed adding records for all required children in their District. As the records were completed using the GAIS system, each record was reviewed by the Principal Investigator. Attempts were made to resolve any unclear information before data cleaning using SAS 9.4 software.
Data Analysis Analyses were done using SAS 9.4 software and macros developed by the Principal Investigator. Data were used to determine what percentage of 7th graders were up-to-date (UTD), which was defined as having at least 3 doses of Hepatitis B, 2 doses of MMR, 2 doses of Varicella, 1 dose of Tdap and 1 dose of MCV4 vaccines. UTD immunization rates as well as individual vaccine coverage rates were assessed at both the state and District levels.
Limitations When interpreting study results, the following limitations of the study should be considered:
1. Compared to other sampling methods, cluster sampling is least representative of the population. Individuals within a cluster tend to have similar characteristics and there is a chance of over- or underrepresentation of the population, leading to skewed results. The two-stage cluster sampling scheme was used because of its low-cost and efficient method of collecting a larger sample size across a wide area.
2. Some IPCs had less than 30 middle schools in their District and were forced to sample all the middle schools in the District. Larger Districts were only required to sample 30 schools, due to time and resource restraints, and therefore many schools were not sampled which may have affected the results of the study.

The 2016 Georgia Adolescent Immunization Study (GAIS) sampled 6,191 adolescents in seventh grade (Table 1).
An adolescent was considered up-to-date (UTD) if they received at least 3 Hepatitis B, 2 MMR, 2 Varicella, 1 Tdap and 1 MCV4 vaccines by the time they enrolled in school. The UTD immunization rate for 7th graders in Georgia was 94.7% for 2016. Overall, females were slightly more likely to be UTD than males. Immunization rates that decreased from the previous year are bolded (Table 1).
Coverage rates among male and female adolescents were similar for all vaccines except HPV. More female adolescents received 1 dose of HPV vaccine than male adolescents (47.4% vs. 41.3%). A higher number of female adolescents received 3 doses of HPV vaccine than male adolescents (16.2% vs 13.3%)(Table 1).
In the overall State sample, 5 adolescents had serologic evidence of immunity-- 3 for Varicella, 1 for MMR, and 1 for Hepatitis B. Statewide, 43 students had either physician-diagnosed or a history of varicella illness (not shown).

Of the 6,191 seventh graders sampled, 49.9% were male, 48.0% were female and 2.1% had missing gender information (Table 2).
Transfer students with incomplete immunization records have a 30-day waiver period to meet immunization requirements. Only 15 (0.2%) of the students in the final sample were found to be in the school "waiver" period, meaning they were incomplete on their schoolrequired immunizations because they were newly enrolled (Table 2).
Within the final study sample, 10 (0.2%) of the adolescents were found to have a medical exemption for one or more vaccinations and 115 (1.9%) had a religious exemption on file (Table 2).

The 2016 GAIS sampled 363 adolescents in seventh grade from District 1-1 (Table 1-1-A).
An adolescent was considered up-to-date (UTD) if they received at least 3 Hepatitis B, 2 MMR, 2 Varicella, 1 Tdap and 1 MCV4 vaccines by the time they enrolled in school. The UTD immunization rate for seventh graders in District 1-1 was 96.1% for 2016. Immunization rates that decreased from the previous year are shown bolded (Table 1-1-A).
Coverage rates among male and female seventh graders were similar for all vaccines except HPV. More female adolescents received 1 dose of HPV vaccine than male adolescents (47.8% vs. 40.1%). A higher number of female adolescents received 3 doses of HPV vaccine than male adolescents (12.4% vs 7.0%) (Table 1 -1-A).
Of the 363 seventh graders sampled from District 1-1, 47.4% were male and 51.2% were female. The gender for five students (1.4%) was unknown or missing (Table 1-1-B).
None of the students in the sample were found to be in the school "waiver" period, meaning that no student was incomplete due to being newly enrolled in school (Table 1-1-B).
None of the adolescents in the District 1-1 sample were found to have a medical exemption for one or more vaccinations. Eight adolescents (2.2%) had a religious exemption on file (Table 1-1-B).

An adolescent was considered up-to-date (UTD) if they received at least 3 Hepatitis B, 2 MMR, 2 Varicella, 1 Tdap and 1 MCV4 vaccines by the time they enrolled in school. The UTD immunization rate for seventh graders in District 1-2 was 90.5% for 2016. Immunization rates that decreased from the previous year are shown bolded (Table 1-2-A).
Coverage rates among male and female seventh graders were similar for all vaccines. (Table 1-2-A).
Of the 326 seventh graders sampled from District 1-2, 51.5% were male and 46.9% were female. The gender for five students (1.5%) was unknown or missing (Table 1-2-B).
None of the students in the sample were found to be in the school "waiver" period, meaning that no student was incomplete due to being newly enrolled in school (Table 1-2-B).
None of the adolescents in the District 1-2 sample were found to have a medical exemption for one or more vaccinations. Seventeen adolescents (5.2%) had a religious exemption on file (Table 1-2-B).

An adolescent was considered up-to-date (UTD) if they received at least 3 Hepatitis B, 2 MMR, 2 Varicella, 1 Tdap and 1 MCV4 vaccines by the time they enrolled in school. The UTD immunization rate for seventh graders in District 2-0 was 94.8% for 2016. Immunization rates that decreased from the previous year are shown bolded (Table 2-0-A).
Coverage rates among male and female seventh graders were similar for all vaccines except HPV. More female adolescents received 1 dose of HPV vaccine than male adolescents (40.1% vs. 29.2%) (Table 2-0-A).
Of the 344 seventh graders sampled from District 2-0, 49.7% were male and 48.5% were female. The gender for six students (1.7%) was unknown or missing (Table 2-0-B).
None of the students in the sample were found to be in the school "waiver" period, meaning that no student was incomplete due to being newly enrolled in school (Table 2-0-B).
None of the adolescents in the District 2-0 sample were found to have a medical exemption for one or more vaccinations. Fourteen adolescents (4.1%) had a religious exemption on file (Table 2-0-B).

An adolescent was considered up-to-date (UTD) if they received at least 3 Hepatitis B, 2 MMR, 2 Varicella, 1 Tdap and 1 MCV4 vaccines by the time they enrolled in school. The UTD immunization rate for seventh graders in District 3-1 was 94.1% for 2016. Immunization rates that decreased from the previous year are shown bolded (Table 3-1-A).
Coverage rates among male and female seventh graders were similar for all vaccines except HPV. More female adolescents received 1 dose of HPV vaccine than male adolescents (51.1% vs. 44.7%) (Table 3-1-A).
Of the 358 seventh graders sampled from District 3-1, 50.0% were male and 49.2% were female. The gender for three students (0.8%) was unknown or missing (Table 3-1-B).
None of the students in the sample were found to be in the school "waiver" period, meaning that no student was incomplete due to being newly enrolled in school (Table 3-1-B).

2016 Georgia Adolescent Immunization Study Report
The 2016 GAIS sampled 362 adolescents in seventh grade from District 3-2 (Table 3-2-A).
An adolescent was considered up-to-date (UTD) if they received at least 3 Hepatitis B, 2 MMR, 2 Varicella, 1 Tdap and 1 MCV4 vaccines by the time they enrolled in school. The UTD immunization rate for seventh graders in District 3-2 was 95.3% for 2016. Immunization rates that decreased from the previous year are shown bolded (Table 3-2-A).
Coverage rates among male and female seventh graders were similar for all vaccines (Table 3-2-A).
Of the 362 seventh graders sampled from District 3-2, 47.0% were male and 51.4% were female. The gender for six students (1.7%) was unknown or missing (Table 3-2-B).
One student in the sample was found to be in the school "waiver" period, meaning they were incomplete due to being newly enrolled in school (Table 3-2-B).
None of the adolescents in the District 3-2 sample were found to have a medical exemption for one or more vaccinations. Seven adolescents (1.9%) had a religious exemption on file (Table 3-2-B).

An adolescent was considered up-to-date (UTD) if they received at least 3 Hepatitis B, 2 MMR, 2 Varicella, 1 Tdap and 1 MCV4 vaccines by the time they enrolled in school. The UTD immunization rate for seventh graders in District 3-3 was 92.8% for 2016. Immunization rates that decreased from the previous year are shown bolded (Table 3-3-A).
Coverage rates among male and female seventh graders were similar for all vaccines (Table 3-3-A).
Of the 293 seventh graders sampled from District 3-3, 54.3% were male and 39.2% were female. The gender for nineteen students (6.5%) was unknown or missing (Table 3-3-B).
None of the students in the sample were found to be in the school "waiver" period, meaning that no student was incomplete due to being newly enrolled in school (Table 3-3-B).
Five (1.7%) of the adolescents in the District 33 sample were found to have a medical exemption for one or more vaccinations. Three adolescents (1.0%) had a religious exemption on file (Table 3-3-B).

An adolescent was considered up-to-date (UTD) if they received at least 3 Hepatitis B, 2 MMR, 2 Varicella, 1 Tdap and 1 MCV4 vaccines by the time they enrolled in school. The UTD immunization rate for seventh graders in District 3-4 was 92.1% for 2016. Immunization rates that decreased from the previous year are shown bolded (Table 3-4-A).
The UTD immunization rate for female seventh graders was higher than male seventh graders (97.0% vs. 92.0%). More female adolescents received 1 dose of HPV vaccine than male adolescents (43.5% vs. 37.7%) (Table 3-4-A).
Of the 353 seventh graders sampled from District 3-4, 49.6% were male and 47.6% were female. The gender for ten students (2.8%) was unknown or missing (Table 3-4-B).
None of the students in the sample were found to be in the school "waiver" period, meaning that no student was incomplete due to being newly enrolled in school (Table 3-4-B).

The UTD immunization rate for seventh graders in District 3-5 was 88.2% for 2016. Immunization rates that decreased from the previous year are shown bolded (Table 3-5-A).
Coverage rates among male and female seventh graders were similar for all vaccines except HPV. A higher number of female adolescents received 3 doses of HPV vaccine than male adolescents (20.2% vs 14.9%) (Table 3-5A).
Of the 374 seventh graders sampled from District 3-5, 53.7% were male and 43.6% were female. The gender for ten students (2.7%) was unknown or missing (Table 3-5-B).
Eleven of the students in the sample were found to be in the school "waiver" period, meaning they were incomplete due to being newly enrolled in school (Table 3-5-B).
None of the adolescents in the District 3-5 sample were found to have a medical exemption for one or more vaccinations. Nine adolescents (2.4%) had a religious exemption on file (Table 3-5-B).

The 2016 GAIS sampled 336 adolescents in seventh grade from District 6-0 (Table 6-0-A).
An adolescent was considered up-to-date (UTD) if they received at least 3 Hepatitis B, 2 MMR, 2 Varicella, 1 Tdap and 1 MCV4 vaccines by the time they enrolled in school. The UTD immunization rate for seventh graders in District 6-0 was 97.0% for 2016. Immunization rates that decreased from the previous year are shown bolded (Table 6-0-A).
Coverage rates among male and female seventh graders were similar for all vaccines except HPV. More female adolescents received 1 dose of HPV vaccine than male adolescents (62.9% vs. 36.8%). A higher number of female adolescents received 3 doses of HPV vaccine than male adolescents (19.5% vs 8.8%) (Table 6 -0-A).
Of the 336 seventh graders sampled from District 6-0, 50.9% were male and 47.3% were female. The gender for six students (1.8%) was unknown or missing (Table 6-0-B).
None of the students in the sample were found to be in the school "waiver" period, meaning that no student was incomplete due to being newly enrolled in school (Table 6-0-B).
