All requests for a determination or Letter of Nonreviewability with regard to Certificate of Need matters should be directed to Clyde L. Reese, III, General Counsel. These may be sent directly to Mr. Reese on the 40th Floor of 2 Peachtree. Only one original copy should be submitted. The Office of General Counsel will attempt to answer all determination requests within 30 days if no additional information is needed from the inquiring party. All requests for Letters of Nonreviewability will be addressed within 60 days if no additional information is needed. All inquiries, pleadings, or other information or material submitted with regard to the Health Planning Review Board should be submitted to Mr. Reese and the Chair of the Review Board at 2 Peachtree, 40th Floor. Information or inquiries with regard to pending Certificate of Need applications should continue to be submitted to the assigned review analyst on the 34th Floor of 2 Peachtree.
Please be advised that June 1, 2002 is the last day the Department will accept old applications currently in circulation. Applicants can download and complete the revised CON application using Microsoft Word. The application is also available in the PDF format for review and copy.
Doris Berry, Compliance Officer, monitors post CON approval procedures, which includes processing the new Post Approval Requirements, including the Progress Report Form. Note that the new Post Approval Requirements replace the old Performance Approval Period Requirements. These documents are also available online at the Department of Community Health's website.
