AMENDMENT TO THE HEALTH PLAN DECISION 
GUIDE 
(July 1  December 31, 2005) 
PLEASE NOTE THE FOLLOWING CORRECTIONS TO THE HEALTH PLAN DECISION GUIDE. 
1. The dollar amounts for the monthly tobacco and spousal surcharges as outlined on page two (2) have changed. The new dollar amounts are as follows: Tobacco Surcharge - $40 Spousal Surcharge - $30 
2. The dates for Open Enrollment have changed. The new dates are: April 22  May 20, 2005. 
3. United Healthcare has expanded their service area to include Baldwin County. 
4. If you participate in the Georgia Merit System Flexible Benefits Program, the hours for www.gabenefits.org will be from 7:00 a.m.  midnight each day.
