 How much do CCSP services cost? Based on Medicaid income limits, some eligible individuals pay some of the cost their services, while others do not. 
 Is there a waiting list for CCSP services? Admission to CCSP is based on available funding determined by the state legislature, so a waiting list may develop. Priority is given to those in most need of services. 
 How long will I be eligible to receive CCSP services? Services are dependent on an individual's continuing to meet CCSP eligibility and service criteria. 
 How do I find out if I'm eligible for the CCSP? Application or referral occurs when you or a family member contacts your local AAA:

 What is the CCSP? The Community Care Services Program (CCSP) supports older and/or functionally impaired consumers to remain in their homes and communities. The CCSP is a Medicaid waiver program that provides community-based social, health and support services to eligible consumers as an alternative choice to institutional placement. 
Who is eligible for services?  Individuals who qualify, or potentially 
qualify, for Medicaid  Individuals who otherwise need 
institutional care in a nursing facility  Screening and assessment 
determine that health and safety needs may be met by services in the home or community  The doctor approves CCSP services. 
What services are available? Eligible individuals may receive a combination of CCSP services and other community resources. The care coordinator assesses appropriateness for CCSP, develops a comprehensive plan of care with the consumer, and arranges for the delivery of service(s). The care coordinator monitors providers of service, the quality of service delivery and ongoing client care. 
Adult Day Health Health, therapeutic, social and recreational activities are provided in a communitybased group day program. 
 
Alternative Living Services State licensed personal care homes provide 24 -hour supervision, medicallyoriented personal care, and health related support services to those unable to remain in their homes. 
Emergency Response Services In-home electronic 2-way, 24 hour communication support system calls for help in an emergency. 
Home Delivered Meals Nutritionally balanced meals delivered to the home. To be eligible, an individual must receive another CCSP service. 
 
Home Delivered Services/ Skilled Nursing Ordered by a physician, provides home health, and includes skilled nursing services, physical/ speech/ occupational therapies, personal support aides, and medical social services to individuals in their homes. 
 
Out-of-Home Respite Care Temporary relief for the primary care giver responsible for full time care of individual.
